Ayushman Bharat card: check eligibility and get your card
How to check whether your family is covered, which documents are asked for, and what to do if your name is missing.

Ayushman Bharat (PM-JAY) gives eligible families health cover for hospital treatment at empanelled hospitals. Eligibility is decided from official lists and criteria, not by any private centre, so the first step is always to check whether you are covered.
If your family is eligible, you can usually complete e-KYC with Aadhaar and download or collect your Ayushman card. If you are not on the list, ask the official helpline or your local health office what applies to you.
Check eligibility
Use the official beneficiary portal or the helpline to search by mobile number or ration card details. Do not pay anyone to ‘add’ your name; the list is maintained by the authorities.
Documents and e-KYC
Keep Aadhaar of the person, a ration card or family details, and a mobile number linked to Aadhaar for the OTP. Only enter the OTP yourself.
Using the card
Carry the card or Aadhaar when you go to an empanelled hospital and ask for the Ayushman help desk. Treatments and limits follow the scheme rules.
